All referrals and subsequent placements of individuals in homes operated by Summertree are made through the above-listed Community Mental Health agencies. When vacancies occur, Community Mental Health staff assess potential candidates and determine the appropriateness for placement in one of the homes or apartments that Summertree operates. Once a referral packet is received from Community Mental Health, the Executive Director, along with the Residential Manager and Home Supervisor review the information received and together make a determination as to the appropriateness of the referral. One of the final steps is to conduct a pre-placement meeting. This typically includes the CMH Supports Coordinator, the parent and/or guardian, the Residential Manager, the Home Supervisor, and of course the consumer.
On occasion, the person being considered for placement visits the home. This visit may last a few hours or even extend to several days depending on the circumstance. An Admission to a home is made after it is determined that the needs of the consumer can be met and the person entering will be a good “fit” with the other residents.
